You’ll need to provide proof of caring as part of your application. You can download our Proof of Caring template, and have it completed by a medical professional, support worker, teacher, the person being cared for, or a counsellor.
You can either print and fill it out by hand, or use a PDF editor like Adobe Acrobat, and submit it with your application.
